Event Information

The tenth annual CCHC Golf Outing is a four person scramble. The event will be held at
Edgewood Golf Club on Friday, July 21, 2017. Registration begins at 11:00 a.m., with a shotgun start at 1:00 p.m. (sharp). The golf registration fee is $90 per person before July 14; after that date, the registration fee is $100 per person. The amount includes green fee, golf cart, lunch, dinner, on course refreshments and prizes. We will accept individual registrations and pair up foursomes. The registration fee for a foursome is $325. The deadline for registration and payment is July 14, 2017.

About the Cause

Central Counties Health Centers, Inc. (CCHC) is an organization committed to providing access to quality, cost-effective health, oral (dental) and behavioral health care to the Springfield, Sangamon County and Jacksonville communities without regard to any socioeconomic barriers including the ability to pay. The vast majority of patients served by CCHC are the working poor, the uninsured and Medicaid populations. Originally founded in 1999, CCHC saw 150 patients in the first month of existence. CCHC currently serves more than 3,000 patients each month. Monies raised from this event will help further the CCHC mission to provide our services to all residents of central Illinois, including the uninsured, homeless, working poor and medically underserved of Springfield and surrounding communities.

How You Can Make a Difference

PREMIUM SPONSORSHIP $5,000 +
Your $5000 donation will designate your company as the Master Sponsor of the golf tournament. Your corporate name and logo will be prominently displayed at registration, throughout the golf course, on all marketing material, at dinner and you will receive voice recognition at the evening meal. You may include promotional materials in starter bags. Your sponsorship also includes tournament entry for two foursomes.

19TH HOLE SPONSORSHIP $ 2,500
Your $2500 donation will provide lunch and dinner for every golfer registered for the event. Your corporate name and logo will be included in all marketing materials, displayed prominently at both meals and at registration and your company will be recognized during dinner. You may include corporate promotional materials in the starter bags. Your sponsorship also includes tournament entry for your foursome.

STARTER BAG SPONSORSHIP $ 1,500
Your $1500 donation will be used to provide items/materials for the starter bags each golfer receives at registration. You will work with the Tournament Director to choose items to be included in the bag with your corporate logo. Your corporate name will also be displayed at registration, in all marketing materials and you will receive recognition at dinner. Your sponsorship also includes tournament entry for your foursome.

GOLD SPONSOR $1,000
Your $1000 donation includes your corporate name and logo displayed at tournament registration table, a large display sign on the 1st and 10th hole tee box areas, table tent or signage display at dinner, corporate promotional material included in starter bags, voice recognition during awards ceremony and tournament entry for your foursome.

SILVER SPONSORSHIP $500
Your $500 donation includes a medium display sign on the 1st and 10th tee box areas, table tent or signage display at dinner, corporate promotional material included in starter bags, voice recognition during awards ceremony and your corporate name on the beverage cart.

BRONZE SPONSORSHIP $250
Your $250 donation includes your corporate name displayed on the tee box area as the sole sponsor of the golf hole and corporate promotional material included in starter bags.

SHARED HOLE SPONSORSHIP $125
Your $125 donation includes your corporate name displayed on the tee box area as a shared sponsor of a golf hole.
