AWARDS:
Four-Mile Run/Walk: Trophies will be awarded for first place overall, and male and female winners. Medals will be awarded to the top three in each age group.

Commemorative Memorial Flags are available for purchase to support Great River Hospice. Flags will be placed around the Great River Medical Center lake on Saturday, November 1, to motivate participants in the Seventh Annual Great River Hospice Fun Run/Walk as they complete the last mile of their race.

Following the race, the flags will become your keepsake for display in your own yard or garden. You may pick up your flag on Nov. 1 or at the Great River Hospice office, 1306 S. Washington Road, West Burlington, Monday through Friday from 8 a.m. - 4:30 p.m., through the end of November.

Flags can be personalized with text only or with text and a photograph. If you wish to have a photograph printed on your flag, you must mail the photograph along with this order form. Please send a copy rather than the original, if possible. Mailed pictures may be picked up at the Fun Run/Walk or at the Great River Hospice office through the end of November.

You are free to be as creative as you like with the personalization of your flag. You might choose to imprint your flag with a favorite quotation or scripture. Or you may choose to take a traditional approach such as this sample wording:
