Waiver and Release

I understand that the FAM Turkey Trot 5K Run/Walk is a physical activity event and as such has various risks associated with it. I have full knowledge of the risks associated with this event and state that I am physically fit to participate and am doing so at my own risk. I hereby waive, release and hold harmless all sponsors, contributors, volunteers and officials associated with this event, from any and all liability, claims and rights for damages growing out of, related to, or arising from participation in this event, including, but not limited to death, injuries, illness and loss to property. If I require medical attention as a result of my participation in the event, I authorize medical personnel to provide medical care as is deemed appropriate by such medical personnel. I grant full permission to FAM Turkey Trot, without limitation or compensation, the unrestricted right to utilize my likeness and name in any form for any purpose. I acknowledge that I have carefully read and understand this waiver and release.
