RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK, AND INDEMNITY AGREEMENT, AND PARENTAL CONSENT AGREEMENT
In consideration of participating in Will Run For Cookies, I represent that I understand the nature of the Event and that I and/or my minor child are qualified, in good health, and in proper physical condition to participate in such Activity. I recognize that this event presents an element of risk. I agree to assume all risks involved in this event and release, waive, discharge, hold harmless, indemnify and covenant not to sue any of the employees, volunteers, sponsors, or any and all agents associated with Dakin Humane Society. I fully understand that the Will Run For Cookies event involves risks of bodily injury and I fully accept and assume all such risks and all responsibility for losses, costs and damages I and/or my minor child incur as a result of I and/ or my minor child’s participation in the Activity. In signing this entry I, for myself and anyone entitled to act on my behalf, waive and release anyone sponsoring, sanctioning, or managing this race and their representatives from all claims or liabilities of any kind arising out of my participation in this event even though that liability may arise out of negligence or carelessness on the part of the persons named in this waiver. If the runner/walker is under 18, I certify that I am the legal guardian or parent and am agreeing to the above terms. Further, I hereby grant full permission to any and all of the foregoing to use my likeness in all media including photographs, pictures, recordings or any other record of this event for any legitimate purpose. I hereby grant full permission to any and all of the foregoing to use my and/or my children(s) likeness in all media including photographs, pictures, recordings or any other record of this event for any legitimate purpose. I have read this release and waiver of liability, assumption of risk and indemnity, and photograph/recording release and fully understand it.
