Release and Waiver: For and in consideration of my (my child’s) participation in the FOLEPI River Trail Classic, and on my behalf (or my child’s behalf) and my heirs, administrators and assigns, I hereby waive, release, covenant not to sue, discharge and hold harmless the City of East Peoria, Perfect Pace, Fondulac Park District, Mickie’s, and all event sponsors from claims, demands, liabilities, loss, damage and causes of action of any sort, including attorney’s fees, for injuries or illness sustained to my (my child’s) person and/or property incurred by reason of or related to my (my child’s) participation in and preparation for this above said event. I certify my (my child’s) participation in this event is free and voluntary. I understand any harassment, intimidation or threats made to race committee members, employees of the City of East Peoria or employees of the Fondulac Park District Administration will result in disqualification from the FOLEPI River Trail Classic and future participation. I have read and understand the foregoing Release and Waiver.
