Event Waiver and Release Form

WARNING: READ CAREFULLY. THIS AGREEMENT INCLUDES A RELEASE OF LIABILITY AND WAIVER OF LEGAL RIGHTS AND DEPRIVES YOU OF THE RIGHT TO SUE THE MOONLIGHT CHASE AND OTHER PARTIES. DO NOT SIGN THIS AGREEMENT UNLESS YOU HAVE READ IT IN ITS ENTIRETY. SEEK THE ADVICE OF LEGAL COUNSEL IF YOU ARE UNSURE OF ITS EFFECT.

I understand there are inherent risks in participating in any road race. In consideration of the acceptance of my entry, I, for myself, my executors, administrators and assignees, do hereby release and discharge, the Moonlight Chase Committee, the Eldridge North Scott Chamber of Commerce, the city of Eldridge, any sponsors, supporters or volunteers, for all claims and damages, demands, actions, whatsoever, in any manner, arising or growing out of my participation in the Moonlight Chase 4-mile/1-mile/1/4-mile run(s). I attest and verify that I have full knowledge of the risks involved in this event, am physically fit, and have sufficiently trained to participate in this event.

If I do not follow all the rules of this event, I understand that I may be removed from the competition. I give my full permission to the Moonlight Chase and their sponsors and corporate sponsors to use any photographs, videotapes or other recordings of me that are made during the course of this event.
