I know that competing in a road race is potentially hazardous activity. I should not enter and compete unless I am medically able and have trained to cover the distance of this event. I agree to abide by any decision of a race official relative to my ability to safely compete in this event and I assume all risks associated with running in this event including but not limited to: falls, contact with other participants, the effects of weather, including high heat and/or humidity, traffic and the conditions of the rad, all such risks being known and appreciated by me. Fully understanding that nature of the activity I hereby, for myself, my executors and administrators waive and release and hold harmless the Columbus Family YMCA, City of Columbus, all sponsors and any of their agents, employees, officers, council members, meet officials, volunteers and workers for any and all rights and statements, and realize this is a contract between myself and the Columbus Family YMCA and is a release of liability. Further, I hereby grant full permission to any and all of the forgoing to use photographs, videotapes, motion pictures, and recordings of me, or any other record of this event, for any legitimate purpose. I sign it of my own free will. If under 18, parent/guardian must sign.
