Event Waiver and Release of Liability

General Participation Waiver: Notice: The City of Cudahy is not responsible for injuries sustained from use of City facilities/fields. Photographers employed by the City may visit our programs and take photos of participants for promotional purposes in City publications. General Release, Waiver and Assumption of Risk: I, the undersigned, understand the potential risk(s) involved in my participation in this physical activity. Dangers could include, but are not limited to cuts, broken bones and other debilitating or permanent physical and emotional injury, damage to personal property, or death. My participation in the City of Cudahy’s 5K may expose me to the risk of personal injury, death or property damage. I hereby acknowledge that I voluntarily participate in this activity and I agree to assume any such risks. I hereby release, discharge, and agree not to sue the City of Cudahy for any injury, death or damage to or loss of personal property arising out of, or in connection with, my participation in the race from whatever cause, including the active or passive negligence of The City of Cudahy or any other participants in the race. In consideration for being permitted to participate in the race, I hereby agree, for myself, my heirs, administrators, executors and assigns that I shall indemnify and hold harmless the City of Cudahy from any and all claims, demands actions or suits arising out of or in connection with my participation in this race. I HAVE CAREFULLY READ THIS RELEASE, HOLD HARMLESS AND AGREEMENT NOT TO SUE AND FULLY UNDERSTAND ITS CONTEXTS. I AM AWARE THAT IT IS A FULL RELEASE OF ALL LIABILTY AND SIGN IT ON MY OWN FREE WILL.
