I acknowledge that my participation in a 5k run/walk could be hazardous and could possibly lead to an injury or death. I certify I am in good health and I assume all risks for myself and my children when participating. I and my children, for myself or anyone on my behalf, waive and release The Bare Hair 501c3, city of Clearcreek Township, The National Alopecia Areata Foundation, Children's Alopecia Project, event sponsors, volunteers, personnel, their representatives or successors from all claims or liabilities of any kind arising out of my participation in this event. I grant permission for my photograph to be used for any legitimate purpose. I understand that this event may be cancelled by conditions outside of the control of event organizers for the safety and well-being of event staff and participants such as severe weather, civil unrest, pandemics, bans on public gatherings, threat of terrorism, or other unforeseen circumstances and I consent that I am not entitled to a refund once my registration is complete.
