Event Waiver and Release of Liability

I am knowingly choosing to participate in an athletic event that involves inherent risks. As a participant in the 5K “Walk the Plank”, I hereby release and agree to hold harmless the Greensburg Community Schools Education Foundation, Inc., the Greensburg Community School Corporation, any sponsors, officials or organizers of this event and each of them together with their successors, assigns, officers, agents, and employees from any and all liability for injuries to person or property suffered by me as a result of my participation in this event and its related activities. The signature(s) below confirm that I assume all risks associated with my participation in the event including, but not limited to, falls, effects of weather, trail conditions, and all risks associated with running/walking in an urban setting.
I verify that I am physically fit, have trained sufficiently for this event and that my physical condition has been verified by a licensed doctor. Furthermore, I agree that, at no time, is it the duty of any of the parties listed above to check, monitor, or evaluate my physical condition relative to participation in this event.
I agree to indemnify any of the parties listed above for damages I have caused to said party(ies) and/or any third parties by my participation in this event.
I agree that the Greensburg Community Schools Education Foundation, Inc. may use any and all photographic images and video or audio recordings made during this event, including those involving myself and/or my minor child.
In addition, I agree to yield to all emergency vehicles on the course and follow directions of the personnel in the event they are needed for treatment of another participant.
