Please read carefully. This is a legal document.
1. Voluntary Participation
I acknowledge that participation in the Origin Story Ultra is voluntary. I understand the physical and mental demands of a 24-hour running event and affirm that I am in good physical condition and capable of participating safely.
2. Assumption of Risk
I am aware that participation in this Event involves inherent risks, including but not limited to:
● Physical exertion and overexertion
● Dehydration, heat stroke, or hypothermia
● Falls, collisions, or accidents with natural or man-made obstacles
● Weather-related hazards
● Wildlife encounters
● Delayed access to emergency medical care
I voluntarily assume all such risks, including those arising from the negligence of the Event organizers, sponsors, or volunteers.
3. Medical Conditions
I certify that I have disclosed any relevant medical conditions that may affect my ability to safely participate. I understand I am responsible for managing my own health and bringing necessary medication (e.g., inhaler, insulin, EpiPen). I understand that emergency medical services may be delayed depending on the location and conditions.
4. Medical Treatment
I authorize the Event organizers and their representatives to provide or seek medical treatment on my behalf in the event of injury, accident, or illness during participation. I release all parties from any liability arising from medical care provided to me. There will be no medical staff on site.
5. Release and Waiver of Liability
In consideration of being permitted to participate in this Event, I hereby release and discharge:
● Brian Wandzilak, its employees, officers, volunteers, and agents
● The City of Lincoln, its departments, officials, and employees
● All Event sponsors, vendors, and affiliates from all liability, claims, demands, or causes of action for any injury, loss, or damage to person or property arising out of my participation, whether caused by negligence or otherwise.
6. Rules and Conduct
I agree to abide by all race rules, safety guidelines, and instructions issued by Event officials. I understand that violations may result in disqualification or removal without refund.
7. Photo and Media Release
I grant the Event permission to use my name, voice, likeness, and image in photos, videos, or media coverage for promotional or informational purposes without compensation.
8. Minors
If the participant is under 18 years old, this waiver must be signed by a parent or legal guardian who agrees to the terms on the minor’s behalf.
9. Severability
If any provision of this waiver is found to be invalid or unenforceable, the remaining provisions shall remain in full force and effect.
