Parental Authorization Form Junior Pirate Athletic League/Intramural Activities

The Phoenix-Talent School District will be offering athletic development opportunities in various sports for students in grades K-6. All students are invited to participate in Junior Pirate Athletic League activities. Practices will be held after the school day.

The school and employees assume no liability beyond that of reasonable caution and care in conducting these activities. To give permission for your student to participate, please fill out the information below.
