Event Waiver and Release of Liability

I agree to self monitor and confirm no signs or symptoms of COVID-19 in past 7 days (OR at least 7 days from onset of any COVID-19 infection signs or symptoms AND at least 72 hours since signs / symptoms have resolved) before the event. I also confirm no close sustained contact with anyone who is sick within 14 days of the event and no travel within 14 days of the event to an international location with widespread ongoing COVID-19 transmission as determined by the CDC or travel on a cruise ship or river boat. I agree not to attend the event if I have a temperature greater than 100.4 F (38.0 C). In consideration of acceptance of my entry, I hereby release, discharge, and agree to hold Seymour Parks and Recreation, the City of Seymour, any sponsors, officials or organizers of this event and each of them together with their successors, assigns, officers, agents and employees from any and all liability for injuries to property or person suffered by me as a result of my participation in this event. By execution of this waiver, I assume all risks associated with my participation in this event including, but not limited to, falls, the effects of weather, traffic, roads and trail conditions, and all risks associated with running/walking in an urban and rural setting whether or not such risks are specifically known or appreciated by me. I verify that I am physically fit and sufficiently trained for competition in this event and that my physical condition has been verified by a licensed medical doctor. I specifically agree that it is not the duty of any of the parties listed above to check, monitor, or evaluate my physical condition for the appropriateness of my participation in this event at any time. I agree to indemnify any of the parties listed above for damages caused by me to any such party or parties and any third parties, by virtue of my participation in this event.
