I know that participating in the Park Glen Turkey Trot 5K Timed Run/Walk or 2K Fun Run/Walk is a potentially hazardous activity. By my signature, I certify that I am medically able and properly trained to participate in this event. I agree to abide by any decision made by race officials. I assume all risks associated with participating in this event including but not limited to falls, contact with others, hazards, traffic, effects of weather, conditions of running surfaces, and medical/physical issues. Having read this waiver and in consideration of the acceptance of my entry, I, for myself, my heirs, executors, administrators and/or anyone acting on my behalf, waive any rights or claims that I may have or that may accrue against Park Glen Neighborhood Association, Inc., the City of Fort Worth, Alliance United Methodist Church, and any and all sponsors, hosts, planners, promoters, facilitators, and workers of this event and release all said parties from liability of any kind. If I choose to participate in this event with a baby, child, or pet, I will be solely responsible for the baby, child, or pet.
