Please enter me in the Colby Smith Memorial 4 Mile Classic. In consideration of the acceptance of my entry, I hereby for myself, my heirs, executors, administrators and assignees, I do hereby release and discharge Aquin Catholic Schools, the Diocese of Rockford, the City of Freeport and any sponsors, volunteers, officials, supporters and property owners for all claims and damages, demands, actions whatsoever in any manner arising or growing out of my participation in the Colby Smith Memorial 4 mile Classic. I attest and verify that I have full knowledge of the risks involved in this event and I am physically fit and sufficiently trained to participate in this event. I also grant permission to all of the foregoing to use photographs, quotations or other records for legitimate purposes. Bib numbers not transferable.
